Story
It has been 10 years since the Code Blue team – Aizawa Kosaku, Shiraishi Megumi, Hiyama Mihoko, Saejima Haruka and Fujikawa Kazuo – made their debut on the medical scene. They have begun to make a career for themselves during this time and taken different paths in their professional and private lives. Aizawa left the emergency centre of Shoyo University-affiliated Northern Hospital and devotes himself to surgeries every day with the aim of further self-improvement as a neurosurgeon. Megumi has been working hard as the staff leader of flight doctors at Shohoku Hospital’s short-handed emergency centre while Mihoko who had once set her sights on becoming a flight doctor, left the hospital and chose to be a gynecologist at a perinatal medical centre. Haruka is still a flight nurse and an ace in the profession. Like Megumi, Fujikawa has also become an emergency doctor specialising in orthopedic surgery and supports Shohoku Hospital’s emergency service. No mention of their growth is complete without Tachibana Daisuke who has been an emergency doctor but is now in a difficult position because of his son. New fellows Natori Soma, Haitani Shunpei and Yokomine Akari arrive at the emergency centre aspiring to become flight doctors, while Yukimura Futaba wants to be a flight nurse. As everyone laments that the four are useless, Megumi has no choice but to attempt to train and make them staff. Meanwhile, Shinkai Hiroki, a strong rival to Aizawa who has finally established himself as a neurosurgeon, shows up. There is a contest in the Department of Neurosurgery to choose a doctor for the prestigious Toronto University.

Characters
Yamashita Tomohisa as Aizawa Kosaku
After his experience as a fellow at the emergency centre, he is now a promising neurosurgeon at Shoyo University-affiliated Northern Hospital and aims to scale new heights. He fights to be the candidate to score a residency at Toronto University which has the world’s best cases and medical care standards in neurosurgery when Shohoku emergency centre unexpectedly receives an offer to take in one of its doctors.

Aragaki Yui as Shiraishi Megumi
She fought on the frontline together with Aizawa Kosaku and the rest as a fellow at the emergency centre and is now the team leader of Shohoku Emergency Centre. To ease the manpower shortage, she has no choice but to make an effort to train the useless new fellows and make them capable although everyone in the emergency centre laments that they are useless.

Toda Erika as Hiyama Mihoko
After overcoming all sorts of challenges as a fellow at the emergency centre, she left to become a gynecologist handling perinatal care. She is still strong-minded and passionate despite the passage of time. When she learns that Mitsui Kanna who once trained her, has left the emergency centre because of her son, she comes back to save the place which is struggling with a shortage of manpower.

Higa Manami as Saejima Haruka
She confronted and overcame the loss of her boyfriend and even continues to with the Doctor Helicopter, or helicopter emergency medical service. She is trusted by everyone and is a role model for a flight nurse. She now has a secret that she cannot tell anyone.

Asari Yosuke as Fujikawa Kazuo
Although he fell far behind the others when he was a fellow, he has found his footing and supports Shohoku Hospital’s emergency service as an emergency doctor specialising in orthopedic surgery. He and Shiraishi Megumi strive to train the new fellows every day. He also continues to watch over Saejima Haruka.

Arioka Daiki as Natori Soma
The only son of a prominent doctor who is the director and owner of a general hospital that became well-known. He possesses the skills for a doctor and yet he feels no passion for medical care. What is the reason for that?

Narita Ryo as Haitani Shunpei
He is earnest and has a kind heart but also an inferiority complex because his skills are inferior. He is not good at dealing with things and also feels terrible about himself for getting cold feet at crucial moments. He has a long way to go to become a strong and dependable doctor.

Araki Yuko as Yokomine Akari
She is proactive and energetic. She has a tendency of being a little dreamy such as her romanticised notions of the medical dramas that she watched as a child which led her to set her sights on becoming a doctor. Despite being overwhelmed by the harshness of what she has to face on site, she overcomes these situations with characteristic cheerfulness and flexibility.

Ando Masanobu as Shinkai Hiroki
He stands in Aizawa Kosaku’s way as his first rival. He is the ace of neurosurgery. He not only has sure skills as a neurosurgeon, he is also good at winning people’s hearts and minds. He fights with Aizawa for the residency at Toronto University.

Shiina Kippei as Tachibana Keisuke
He guided and cultivated Aizawa Kosaku, Shiraishi Megumi, Hiyama Mihoko, Saejima Haruka and Fujikawa Kazuo, and became their beacon. Although he and his wife Mitsui Kanna divorced, they later reconciled. The couple now have to confront their son’s problem and face a big ordeal.

Ryou as Mitsui Kanna
As a flight doctor, she continues to watch over the fellows. She and her husband Tachibana Daisuke are in a difficult position regarding their son.
