Ministry of Health, Labour and Welfare official Shiratori Keisuke (Nakamura Toru), who has come to the hospital to visit his university teacher, learns about the anonymous letter and guesses that the sender is someone who hates Hayami or a hospital staff who wishes to keep Hayami at Tojo University Hospital. The next day, just as the anonymous letter had warned, a patient warded in ICU for a bone fracture develops blood poisoning because of bacterial infection and suddenly takes a turn for the worse. The patient survives, but his life remains in the balance. At that moment, the head nurse, Hanabusa Miwa (Shiraishi Miho) examines an odd item which had been placed in Hayami’s room.
